If you have any questions or need assistance at any time, please don’t hesitate to reach out—we’re always happy to help. We truly look forward to meeting you and caring for you at your first visit.
Appointments
We see patients by appointment. If you need to cancel or reschedule, we kindly ask that you notify us at least 24 hours in advance so we can offer that time to another patient in need.
Arrival
Please plan to arrive 15 minutes early. Be sure to bring a valid government-issued ID (such as a driver’s license), your insurance card, and a list of any medications you are currently taking, including prescriptions and over-the-counter items.
Medical Records
If you have any recent test results, X-rays, or relevant medical records, please bring them with you to your appointment.
Payments
All co-payments, deductibles, and non-covered services are due at the time of your visit.
Prescription Refills
When requesting a refill, please have the medication name, dosage, and your pharmacy’s name and phone number available. Kindly allow up to 24 hours for your request to be reviewed and processed. Please note that refill requests are not handled after hours or on weekends unless it is an emergency.
Insurance
We accept most major insurance plans. We recommend calling our office ahead of time to confirm your specific coverage. Please bring your insurance card with you, as we will need a copy for our records. If your plan requires a referral from your primary care physician, be sure to obtain it prior to your visit.
